Human Services:
- Provide consumers with individual counseling during regularly scheduled sessions and crisis situations, including suicide assessments.
- Facilitate therapy in a group modality with consumers diagnosed with a mental health condition.
- Conduct intakes for the Continuing Day Treatment Program and the Homeward Bound Program.
- Locate and develop resources relevant to consumers' needs.
- Complete psychosocial assessments with consumers.
- Provide phone consultation on how to access services within the mental health system in NYC.
- Advise callers on appropriate referrals to resources and housing programs that match consumers' needs and preferences.
- Train staff and consumers on accessing supportive mental health housing.
- Advocate at the local and state level on a variety of mental health issues.
- Acted as CUCS Housing Resource Center's Site Representative to the CUCS' Advocacy Committee (2002-2004)
- Facilitate community meetings.
- Interview housing providers on the services they offer.
- Write the "Information Update" article in CUCS' Vacancy and Information Update and gather the data for vacancies within NYC.
- Provide administrative assistance to referring workers and housing providers regarding the Single Point of Access (SPOA) Project.
- Act as the central contact coordinating benefits, entitlements, residential and program needs, and other services for consumers with mental retardation / developmental disabilities, traumatic brain injuries, mental health condition, substance abuse, MICA, senior citizens, medical needs, and/or homelessness.
- Interview consumers to assess for their needs, goals, preferences, and their psychiatric and medical stability to ensure appropriate services are offered to enhance their lives.
- Develop individualized service plans and treatment plans that reflect consumers' goals and growth to assure appropriate delivery and maintenance of services.
